Abstract
Ankle sprains are frequent injuries that constitute a major cause of work disability. The sprain of the lateral complex is the most frequent one and is associated with a mechanism of inversion and plantar flexion of the foot. The initial approach should include a medical history that determines the mechanism of trauma and that considers the risk factors in order to obtain a timely diagnosis. Currently, conservative management of ankle sprains is the standard of care because of its better cost-effectiveness.
Method: For the elaboration of this paper a search of literature was carried out excluding papers older than 5 years, except those that constituted clinical guidelines from which the most recent ones available were included. The terms “ankle sprain”, “ankle sprain management” and “ankle sprain prevention” were sought, both in Spanish and English.
Conclusions: The recommended initial therapy is the RICE strategy (rest, ice, compression, elevation). NSAIDs are an acceptable option in early stages. A compression bandage allows to manage edema and it is also reasonable to use additional support measures according to severity of the injury, such as anklets or semi-rigid supports. The approach should be mobilization and early rehabilitation, with a prospective follow-up that allows to prevent complications such as chronic joint instability and recurrent sprain.
