Resumen
El artículo presenta una revisión narrativa sobre el manejo del dolor mediante el uso seguro de medicamentos durante el embarazo. Es una actualización sobre el estado actual sobre el manejo farmacológico del dolor en embarazadas. Se revisaron más de 20 estudios recientes en el ámbito clínico, publicados en plataformas de importante difusión como Chocrane, Medline, ScienceDirect, Springer, entre otras. Se describen los enfoques sobre el tratamiento del dolor en la embarazada. Se hace una descripción general sobre el manejo no farmacológico, con énfasis en la terapia física. Se describe de forma amplia los principales medicamentos seguros para el manejo del dolor en el embarazo. Se destaca el uso del paracetamol como principal recomendado en los dos primeros trimestres. Los opioides de débil efecto se aconsejan en todo el proceso en especial la buprenorfina. En la migraña se recomienda la metoclopramida y los triptanos. El manejo del dolor debe hacerse con criterios rigurosos, tomando en cuenta el historial de la paciente y la idoneidad comprobada del medicamento para la etapa de embarazo. Se debe valorar los posibles riesgos frente a los beneficios esperados.
